Acknowledgement/Payment

PTP Consultancy will acknowledge receipt of this booking form by return, either by fax or email. It is your responsibility to ensure that PTP Consultancy receives payment in advance of the workshop. We will send you further details for the workshop when we receive payment. If you have not heard from us 10 days before the workshop, please contact the PTP Consultancy office.

Payment by invoice without purchase order number

We will accept written confirmation of your booking, on official headed paper and can raise an invoice for you.

Cancellations

Cancellation of the workshop made 28 days before the workshop date will be charged at 10% of the invoice total. Cancellations made between 27 and 15 days before will be charged at 50% of the invoice total. Cancellations made less than 15 days before will be charged the full invoice total. Cancellations must be received in writing. Substitutions for delegates on the day are made at the client's discretion.

The personal information provided by you may be held on a database by PTP Consultancy. If you do not wish your details to be held on our database please write to PTP Consultancy, 41 Harcourt Road, Redland, Bristol BS6 7RF.
